In accordance with ACA reporting requirements, employees may access their 2025 Form 1095 electronically through the Prism employee portal. This form provides information about health coverage offered during the 2025 calendar year.
To view your form, log in to Prism and navigate to your tax menu. You may download or print a copy for your records.
